The clinical Inpatient Transplant Coordinator is a professional registered nurse and a designated member of the transplant team whose primary responsibilities are directly involved with coordinating the clinical aspects of care. The coordinator works with the recipient and their families beginning with the evaluation for donation and continuing through and after transplantation, in a compassionate and tactful manner in order to facilitate access to and provide continuity of care. The coordinator also works with the other members of the transplant team including physicians, surgeons, nurses, social workers, dietitians, financial coordinators and administrative personnel.Coordinates the overall evaluation process for potential recipientsAssesses potential recipient and family/caregiver perception of physical and psychosocial implications of transplantation.Is responsible for listing the recipient on UNETActively participates in transplant program community Outreach efforts.Attends and participates in the pre transplant education sessions and pre and/or post:transplant clinic as assigned.Works effectively with medical and surgical program directors, inpatient units, outpatient clinics and department personnel to optimize patient and family centered carePerforms physical assessment if requiredFollows through, in a timely manner, with orders and protocols for diagnostic studies for recipients.Assesses diagnostic study results, makes appropriate referrals and coordinates care with other disciplinesProvides patient and family educationContributes to the education and acts as the resource person regarding transplantation for all staff nurses, referring physicians and outside municate effectively with referring physiciansParticipate in research where appropriate.Attend transplant conference and participate in the patient selection and problem solving related to patient care and the department.Work efficiently and effectively with Electronic Health Records and database systems.Take call for organ offers and after hour calls for patientsTriages patient problems over the phone according to established protocols, state regulations and own assessment skills.Coordinates the transplant process to include organ acceptance from local OPO and import organs, compatibility verification, patient notification, assist with OR arrangement.Understands the United Network of Organ Sharing (UNOS) policy as it relates matching of donor and recipients and organ acceptance.Provide continuum of care in the office, unit, and clinic during absences of other coordinators.Provide public and professional education where requiredActively involved in quality assessment and process improvement activitiesParticipates in quality metrics and data collection as neededSubmits data for listing patient on UNOS waiting listParticipates in multidiciplinary roundsCoordianates inpatient plan of care with the multidiciplinary teaDocuments patient results in the electronic medical recordCommunicates with pharmacy/pharmacist for discharge medications/ billingEducates patient and family post:transplantAssists patient in appropriate medication box usePrepares patient education packets for post recipientsVerifies patient home medicationsCoordinates transition of care between inpatient and outpatientCommunicates with dialysis units post:transplant (if applicable)Coordinates with referring physicians post:transplant (if applicable)Prepares patients for discharge and outpatient follow:upCompletes regulatory forms and patient formsAssumes lead in directing responsibility of all patient and family educationAssists with triage of pot patient calls during office hours and handles appropriately according to established protocolRequired: Graduate of an accredited Nursing Program with Associates Degree, Nursing Diploma, Bachelors in Nursing, or Masters in Nursing. Valid RN License, minimum of 4 years of Registered Nursing experience. Critical/Acute Care and/or End Stage Organ Disease, and/or
